Combined Notifications for Threads.

If multiple people comment in a thread or something now, it will appear as one notification, which should help if a thread you like or watch gets posted in a lot.

This apparently caused a bug as it made generating notifications take longer. I've modified the code, and made a few more optimizations that I noticed were necessary as a result, and hopefully this should work better now.
