In my field, probably accidents during a construction inspection, a bad design or a purchase fraud for different types of materials, malfuction or misconception of sanitary installations, miscalculations of structures' free-body diagrams, poor management of the estimated budget, etc.

I think as long as its "right." uh-oh trigger word for another drawn-out discussion O.o
Short answer->I believe as would be a general rule of thumb: creation made with minimalist risk or harm to any persons(depending on application, M-16 is obviously designed to kill) and obviously will have an overall positive impact with what is designed rather than drawbacks.
