Hourou Musuko: Wandering Son has a central plot dealing with gender. A boy who wants to be a girl and a girl who wants to be a boy. It's got alot of feels. I haven't finished it yet but I've enjoyed it so far.

Hourou Musuko is definitely the first to come to mind. It knows what it's talking about.
And while SideM (Idolmaster's male series) is pretty generic, I read a Japanese interview during its conception that Saki was made to reflect a more realistic cross-player idol compared to Ryo. They went to a popular LGBT pride spot in Shinjuku and did actual research on transgenders/transvestites.
