Hmm...that's difficult. I'm not sure about anime, but there were some traumatic and seriously messed up deaths in the Corpse Party games. Everything from guillotined by a piano string to having your insides ripped out by 4 ghost children and everything in-between...it was a good game, but still...

Golden Time. Don't care what people say, this was a great anime with some of the most realistic relationship quarrels I've ever seen.
